Success Story: Sammie H.

Sammie H. has been a resident at Fountainview Nursing and Rehab since 2017. Recently, Sammie was hospitalized after experiencing respiratory failure. Following her return to the facility, Sammie demonstrated difficulty with swallowing solid foods, completing safe transfers with staff, walking, and maintaining general balance.

Sammie was evaluated by our Physical, Occupational, and Speech therapists and each discipline created an individualized plan of treatment designed to help Sammie return to her previous level of independence. Sammie’s personal goals were to improve her ability to safely walk with staff, as well as to safely eat and drink anything she wanted. Sammie worked very hard with the therapy team and received therapy 5 times each week for 10 weeks. It seemed to be a long road for Sammie, but with the help of her therapists as well as her personal persistence, Sammie achieved each of her goals.
