Success Story: John E.

John E. was admitted to Fountainview Nursing and Rehab due to acute decline secondary to exacerbation of COPD. John was experiencing general significant weakness and was unable to walk, complete ADLs, or transfer independently.

John was evaluated by our Physical, Occupational and Speech therapists, and a specialized plan of treatment was developed to target his individual needs and his personal goal of being able to regain his independence. John participated in PT, OT, and ST services 5x each week. During therapy, John worked hard to improve his strength, endurance, standing balance, and ability to walk long distances. As John improved, he was also able to complete daily self care tasks without assistance from staff. John worked with the therapists until he and his family felt comfortable with his transition to the assisted living wing. John discharged from skilled therapy services on 7/13/17, and has reached his goal of regaining his personal independence.

John was an absolute joy to work with! The department was always impressed with his dedicated and work ethic. It was a privilege to help him reach his goals.
