Short Term Therapy Success

Sharie “Jo” C. was admitted to Fountainview Nursing and Rehab following a fall resulting in a closed fracture to the lower right leg. Jo was unable to bear weight through her fractured leg, and demonstrated significant weakness which negatively impacted her transfers, self care, and general functional mobility. Jo came to Fountanview after receiving therapy at a different facility in hopes that she would be able to achieve her goal of returning home to live independently.

Jo was evaluated by our Physical and Occupational therapists, and an individualized plan of treatment was developed to target Jo’s deficits as well as personal goals. Jo participated in PT and OT services five times each week targeting strength, endurance, balance, techniques for self care, and compensation strategies for a safely transitioning to her apartment. Jo worked extremely hard during each session. At the time of discharge, Jo had been released to walk distances of up to 200 feet with her walker, she could independently complete dressing and hygiene tasks, and she demonstrated the ability to independently complete financial and medication management. Jo was discharged from therapy as of 11/21/17, and transitioned to live independently at home.

We are tremendously proud of Jo’s progress and it was truly our pleasure to help her obtain her goal of returning to her apartment.
