Short Term Therapy Success

Frances H. was admitted to Fountainview Nursing and Rehab following multiple falls as well as generalized weakness. After our therapy team completed comprehensive evaluations, it was determined that Frances was experiencing difficulty completing self care, transfers, and functional ambulation tasks (e.g., walking from chair to bathroom, or bed to chair, etc. . ). Additionally, Frances reported having trouble “remembering things like she used to.”

Frances was evaluated by our Physical, Occupational and Speech therapists, and an individualized plan of treatment was developed to target Frances’s needs and personal goals. Frances participated in PT, OT, and ST services 5x each week targeting strength, endurance, balance, techniques for self care, and compensation strategies for memory. Frances demonstrated excellent progress while participating in therapy. In 6 weeks, she was able to reach her highest level of functional independence for our current setting, and is now able to independently complete self care tasks, walk up to 100 feet, and rely on strategies to improve her memory throughout each day.
